Benefits Summary

Below is a list of benefits offered to employees of Developmental Services of Nebraska, Inc.  Open enrollments and benefit eligibility are communicated through a variety of channels such as notices sent at payroll time, through e-mail, posted at locations, presented at staff meetings, and announcements posted on the team site.   

Employees must have continuously been scheduled according to the most recent Personal Action Form (PAF)AND working 20+ or 35+ hours to qualify for the benefits listed below.  Dropping below 35 hours more than twice in a calendar year may result in the loss of certain benefits and a new waiting period.  Holiday and Leave time may be used for those eligible to keep their weekly Friday to Thursday time above 35 hours.

Benefits Available To All Employess

Premium Membership Banking – As an employee of DSN, personal bank accounts may be eligible for special packaging options through Wells Fargo.  Contact Wells Fargo and tell them you are an employee of DSN for more information.

Employee Recognition Program – DSN values and appreciates all of the hard work and effort put forth by employees through various formal and informal Employee Recognition formats.  A committee dedicated to employee recognition meets regularly to continuously improve the program.  Contact the Central Office at (402) 435-2800 for current information.

Supplemental Insurance - For pre-tax premiums, this benefit is available first of the month following the successful completion of 3 months at full time status, or at open enrollment for plan year beginning January 1.  Benefits may remain with the employee if you are no longer an employee of DSN.  Employees must sign to enroll or waive this benefit.  Our representative will set up an appointment to go over the options if you are interested.  Rates: Vary by benefit and options selected.

Benefits Available To Employees At 20+ Hours (Includes Previous List)

Tuition Reimbursement – DSN offers after tax Tuition Reimbursement to all employees scheduled and working at least 20 hours per pay week.  The maximum reimbursement per employee is $1000 per calendar year; $500 per term.  The class must be related to future or current employment opportunities within DSN.  Employees seeking reimbursement must first have the class pre-approved and have the complete packet of required information turned in and approved before the class begins.  Class eligibility and reimbursement amount may vary based on five (5) and (10) year anniversary milestones.  

Retirement (401k) – Available to employees the first quarter after completing 12-consecutive months of employment having worked a minimum of 1000 hours (approximately 20 hours per week).  Future quarterly meetings are optional for eligible employees to make changes or obtain more information about the plan.  Open enrollment meetings occur at 1:00 pm the second Monday of the month preceding each quarter’s beginning of January, April, July, and October.  The advisor will rotate which area the information is presented from with video conferencing available in all areas.  Employees must sign up to enroll or waive this benefit or they will automatically be enrolled in the plan up to 2% of their salary.  Employees may choose to participate in a Roth option.  Rates:  employee chooses contribution as an amount or percentage of income up to the maximum amount allowed by the IRS.  DSN contributes $.50 for every $1 up to 6% of the salary put in by the employee.

Benefits Available To Employees At 35+ Hours (Includes Previous List)

Major Medical Benefit – Available the first of the month following the successful completion of 3 months at full time status; after that upon the occurrence of a major life event (loss of coverage, marriage, birth, divorce), or at open enrollment for plan year beginning September 1.  DSN pays a portion of the premiums and offers a pre-tax group discounted rate to employees.  Employees must sign to enroll or waive this benefit.  Current employee rates per bi-weekly pay period: EE Only $50.00; EE + Child(ren) $128.23; EE + Spouse $151.67; Employee + Family $253.92.
Along with the health insurance is a $50,000 life insurance and long term disability plan which covers up to 60% of salary if a qualified event occurs at no additional cost to employees. 

DDental/Vision Benefit - Available the first of the month following the successful completion of 3 months at full time status; after that upon the occurrence of a major life event (loss of coverage, marriage, birth, divorce) or a certificate of creditable coverage and/or waiting periods may apply.  DSN offers a pre-tax group discounted rate to employees.  Employees must sign to enroll or waive this benefit.  Current employee rates per bi-weekly pay period:  EE $10.91; EE+1 $21.58; EE&2+ $34.97.

Section 125 Flexible Spending Account - Available the first of the month following the successful completion of 3 months at full time status or at open enrollment for plan year beginning January 1.  Allows an employee to receive a tax break for qualified medical or daycare expenses and utilizes a Flexible Spending Account debit card option.  Employees must sign to enroll or waive this benefit.  Current employee rates per bi-weekly pay period: Depends on individual up to a maximum of $5000 for both medical and daycare expenses.

Leave Time – Full time employees receive 8 holidays per year, New Year’s Day, President’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day; personal leave according to the attached schedule; and up to 8 hours of bereavement leave with approval.  Leave hours may be used after a 3 month period of going full time and accrue on all hours except overtime.  Leave hours should be used to keep full time employees up to their listed hours on file at or above 35 hours per pay week (Friday to Thursday) in order to maintain qualified benefits.  Hourly employees may work and claim leave hours for the amount of time actually worked up to 8 hours on the Agency observed Holidays listed above only.  For example, an employee who worked 10 hours on the 4th of July may request to get paid additional Leave time in addition to the pay earned working for 8 of those hours.  If 6 hours are worked, 6 Leave hours may be claimed in addition to the pay received for working those 6 hours. 

When requests are submitted for time off, staff will receive Leave time up to the amount of hours on record.  Holiday hours accrued from January 1, 2007 through June 30, 2007 must be used by April 1, 2008 or the hours will be lost.  Leave hours may be banked up to 240 hours.  A bank is available for employees to donate Leave time to other employees for use on qualified Family Medical Leave absences; personal banks will be used before donated bank hours.  Individuals wishing to use or donate hours should fill out a Donation and request of Leave or Holiday Hours Form.  Hours will be allotted equally among all employees on FMLA and are not a guarantee of coverage.
